Participants on SEGF field trips are required to provide the following information to the trip leaders: Travel information, home contact information, health and insurance verification, electronic equipment serial numbers, passport number, etc. The forms below specify the details. We MUST RECEIVE all the requested information as soon as possible and no later than February 8, 2008. NO EXCEPTIONS. The information will be kept confidential and destroyed after the trip.
